The McGill University Health Centre (MUHC) invites you to a Café scientifique to discuss the management of medications from hospital to home, on Wednesday, May 23rd from 6 p.m.until 8 p.m. in the relaxed atmosphere of Java U, 5620 Côte-des-Neiges. This event is funded by The Canadian Institutes for Health Research (CIHR).

Managing medications from hospital to home: can we do better?
How might incomplete information about your medications lead to complications? Frequent transfers of patients between care settings and the increasing number of medications taken by patients can cause adverse drug events. Can electronic tools prevent such errors and make a difference?
